Nurse's Corner

Hello CSG Families!  My name is Jen Salamon and I am the school nurse for the upcoming 2019-2020 school year at Columbus School for Girls.  I have been a parent at CSG for the past four years (Emma graduates in '21 and Allie graduates in '24) and the nurse since January of 2018, and I have thoroughly enjoyed getting to know you as parents, and of course your daughters!  I can’t wait to meet the new students and their parents as well! 

I hold a Bachelor's Degree in Science and Nursing and am a Registered Nurse with a background in Intensive Care and Open Heart Recovery.  My office is room number 173, and is located down the main hallway to the right, just beyond the desk at the Athletic Entrance and adjacent to the dining hall.  I am in my office every weekday from approximately 8:00 am until 4:00 pm and am able to provide basic medical care for acute and chronic illness and basic first aid for injuries.  Other responsibilities include the administration of all prescription and over-the-counter medications, monitoring and assuring immunization compliance, monitoring and assuring care plan and medication compliance for students with asthma, severe allergies, diabetes, and/or other chronic illness, dismissal of students due to illness, as well as conducting vision and hearing screenings.  

My goal is to ensure your daughters’  health and safety while they are at school, and as such, open communication and collaboration with you is a priority.  Please do not hesitate to contact me with any questions or concerns at any time during the year. I love my job, I love interacting with and getting to know your daughters, and I am excited to be a part of this amazing team of educators, faculty, and staff.  I am very much looking forward to a happy and healthy and fun 2019-2020 school year for all of your daughters!

Final Forms

Final Forms is the electronic platform that CSG uses to maintain pertinent student information, including health-related information.  All Forms are to be completed and/or updated and electronically signed annually PRIOR to the first day of school.  The following health-related items must be completed to ensure your daughters’ health in case of an emergency:

  • Contact Information (and emergency contact numbers)

  • Health History and Medical Profile 

  • Medications (current meds taken and over-the-counter permissions)

  • Past and Ongoing Health Conditions (severe allergy, asthma, diabetes, seizure disorder medication and care plan information)

  • Emergency Medical Authorization (granting or declining consent for treatment of your daughter in an emergency)

Immunization Information

As mandated by the State of Ohio, updated immunization records or immunization exemption documentation (if exemption documentation is not already on file) must be provided to the school nurse before the 15th day of the school year for continued school attendance (Ohio Revised Code Section 3313.67).  


Immunization records may be emailed to Jen Salamon, School Nurse, at jsalamon@columbusschoolforgirls.org, faxed to CSG at 614-252-0571, or mailed directly to the school at 65 S. Drexel Avenue, Columbus, OH, 43209. 

CSG Allergy Policy

Columbus School for Girls is a nut free environment.  All food products as well as other health and wellness products (e.g. lip balms, hand sanitizers, lotions) and/or materials brought into the school must be peanut and tree nut free.
