Child Medical Statement

If your daughter will need to take prescription medications at school, a Physician Request for the Administration of Prescription Medications by School Personnel form must be completed by her physician, signed by both physician and parent/guardian, and turned in to Jen Salamon, School Nurse, at jsalamon@columbusschoolforgirls.org, faxed to CSG at 614-252-0571, or mailed directly to the school at 65 S. Drexel Avenue, Columbus, OH, 43209.

If your daughter has severe allergies or reactions and will require an Epi Pen at school - even if she is authorized to carry and administer her own epi pen - a Food Allergy & Anaphylaxis Emergency Care Plan/Authorization for Possession and use of an Epinephrine Autoinjector form must be completed by her physician, signed by both physician and parent/guardian, and turned in to Jen Salamon, School Nurse, at jsalamon@columbusschoolforgirls.org, faxed to CSG at 614-252-0571, or mailed directly to the school at 65 S. Drexel Avenue, Columbus, OH, 43209.

If your daughter is authorized to carry and administer her own epi pen, she is responsible for carrying her epi pen with her at all times.  If your daughter is not authorized to carry and administer her own epi pen, two epi pens with valid expiration dates must be delivered to the nurse’s office prior to the first day of school.

If your daughter has Asthma, and will require and inhaler at school - even is she is authorized to carry and administer her own inhaler - an Asthma Care Plan must be completed by her physician, signed by both physician and parent/guardian, and turned in to Jen Salamon, School Nurse, at jsalamon@columbusschoolforgirls.org, faxed to CSG at 614-252-0571, or mailed directly to the school at 65 S. Drexel Avenue, Columbus, OH, 43209. 

If your daughter is authorized to carry and administer her own inhaler, she is responsible for carrying her inhaler with her at all times.  If your daughter is not authorized to carry and administer her own inhaler, an inhaler with a valid expiration date must be delivered to the nurse’s office prior to the first day of school.

If your daughter has Diabetes, a Diabetes Care Plan must be completed by her physician, signed by both physician and parent/guardian, and turned in to Jen Salamon, School Nurse, at jsalamon@columbusschoolforgirls.org, faxed to CSG at 614-252-0571, or mailed directly to the school at 65 S. Drexel Avenue, Columbus, OH, 43209. 

Any diabetes medical supplies may be kept in the nurse’s office, and may be dropped off any time prior to the first day of school.

If your daughter has seizure disorder, a Seizure Action Plan must be completed by her physician, signed by both physician and parent/guardian, and turned in to Jen Salamon, School Nurse, at jsalamon@columbusschoolforgirls.org, faxed to CSG at 614-252-0571, or mailed directly to the school at 65 S. Drexel Avenue, Columbus, OH, 43209.

The school nurse conducts vision and hearing screenings every year on students in the PYC, Form I, Form III, Form V, Form VII, Form IX, and Form XI.  All screenings are conducted according to the Ohio Department of Health guidelines and requirements. These screenings are important as early detection of vision and hearing problems expedites early treatment, and problems can most often be easily corrected.  This can facilitate better language development, speech, reading, and learning. The school nurse will notify parents of any screening results that indicate that a physician referral is necessary. If your daughter plays sports for CSG, an annual sports physical must be obtained and a Preparticipation Physical Exam Form must be completed by her physician, signed by both physician and parent/guardian, and turned in to Jen Bunker, Assistant Athletic Director, at jbunker@columbusschoolforgirls or Karlyn Botzman, Athletic Trainer, at kbotzman@columbusschoolforgirls.org.  Forms may also be faxed to CSG at 614-252-0571, or mailed directly to the school at 65 S.
